I am a Licensed Master Social Worker (mental health practitioner), experienced with therapeutic intervention with children and teens in multiple private and group settings. 
I recommend the Anxiety Workbook for Teens, by Lisa M. Schab, LCSW to use with this class. This journal-style workbook is not required, but is helpful. I will list the exercises that students using this book should do each week to correlate to our classwork.
