Course Description:

This 6-week course is designed for students with disabilities who are preparing for life after school. Through a combination of self-paced activities, reflection, and guidance, students will create a personalized transition plan that focuses on their goals and aspirations in the areas of education, employment, independent living, and community participation. The course will help students take ownership of their future by breaking down the transition planning process into manageable, actionable steps. By the end of the course, students will have a comprehensive plan that includes long-term and short-term goals, a detailed action plan, and identified support systems.

What Will Be Taught?

Transition Planning Basics: Understanding the importance of early transition planning and the key areas (education, employment, independent living, and community participation).
Self-Awareness: Reflecting on individual strengths, interests, and preferences that will inform the transition plan.
Goal Setting: Learning how to set long-term and short-term goals using the SMART goal framework (Specific, Measurable, Achievable, Relevant, Time-bound).
Action Plans: Breaking down goals into smaller, actionable steps with timelines and identifying the support needed to achieve them.
Support Systems: Identifying people, resources, and community services that will help students achieve their transition goals.
Finalizing the Plan: Putting together all the pieces into a comprehensive and personalized transition plan.

How Is the Class Structured?

This 6-week course is divided into weekly modules, each focusing on a specific aspect of transition planning. Each week, students will engage in:
Reading Materials: Short readings or video clips introducing the key concepts for the week.
Reflection Exercises: Activities that encourage students to reflect on their strengths, interests, and goals.
Goal Setting and Action Planning: Structured worksheets to guide students through setting SMART goals and creating action steps.
Weekly Assignments: Students will submit their work each week, allowing them to track progress and refine their plans.

The course is designed to be self-paced, with one module to complete each week. Students will be given flexibility to work at their own speed, but it’s encouraged to complete each module on schedule for a continuous learning experience.

How Will You Teach?

The course will be taught using a combination of:
Guided Lectures and Videos: Short, engaging video clips and written materials explaining key concepts.
Interactive Worksheets: Hands-on activities that help students reflect, set goals, and create action plans.
Self-Reflection Exercises: Encouraging students to think critically about their strengths, challenges, and aspirations.

My teaching style is supportive, structured, and interactive. I aim to make complex concepts easy to understand by breaking them down into manageable steps. Each student will feel empowered to take charge of their future while receiving clear guidance throughout the process. I focus on a student-centered approach, encouraging students to explore their own ideas and solutions while providing resources and suggestions when needed.

How Much Will Learners Get to Interact with You?

Video Lessons: Each module includes an instructional video where I explain key concepts, share examples, and provide encouragement.
Bi-Weekly & Weekly Check-Ins: After completing the weekly assignments, students will have the opportunity to submit their work for feedback. I will provide personalized comments and suggestions to help them refine their plans.
Discussion and Reflection: Students will have the chance to reflect on their progress and share their thoughts or questions through weekly prompts. I encourage students to share their ideas and goals to foster a collaborative learning experience.
Support and Guidance: Throughout the course, students can reach out to me with specific questions via email or the course platform’s messaging system. I am committed to providing ongoing support as students work through their transition plans.

Learning Needs
This class is designed for students with disabilities. Creating a transition plan unique to the individual is imperative for successful postsecondary outcomes. The class is designed with differentiation in mind.
Parental Guidance
Self-Reflection: Some students may feel overwhelmed or uncertain when reflecting on their strengths, challenges, or future goals. Encouraging students to approach these activities at their own pace can help minimize any potential discomfort. External Resources: During the course, students may be encouraged to access community resources, such as vocational programs or support services, which may involve reaching out to local organizations. Parents should be informed about the types of resources available in their area, particularly if there are any regional differences in services for individuals with disabilities.

All materials learners will need to complete the course will be supplied at the beginning of each topic. A printer is recommended to print and complete the sections of the transition plan.

Teacher expertise and credentials
I bring a rich background in special education, with over 15 years of experience working with individuals with disabilities, both in educational settings and through direct support services. My academic foundation includes a Bachelor's, Master's, and Doctorate in the field of Special Education. My doctoral research focused on evidence-based practices for teaching young adults with disabilities, and I have specialized training in developing individualized transition plans for students with moderate to severe disabilities. My thesis focused on the postsecondary outcomes for individuals with intellectual disabilities.

I have extensive experience in the classroom, having worked with students of varying abilities, including those with moderate to severe disabilities, in both K-12 settings and adult support programs. I have presented at numerous professional conferences on best practices for working with children and adults with disabilities, with a particular focus on behavioral management, transition planning, and life skills development.

In my professional career, I have worked closely with families and educators to develop and implement individualized education plans (IEPs), including transition plans for students with disabilities. I understand the unique challenges that students face when preparing for life after school and have dedicated much of my career to creating actionable, realistic transition plans that set students up for success in employment, education, independent living, and community participation.

Through this course, I aim to pass on my knowledge and experience to students and families, helping them to navigate the transition planning process with confidence and clarity. My goal is to make sure that every student is equipped with the tools they need to succeed after school, whether that means entering the workforce, pursuing further education, or developing greater independence.
